Set Expectations

GUIDE YOUR PATIENT’S EXPECTATIONS, FROM THE VERY BEGINNING

It is important that you support your patient’s treatment success by giving them sufficient
knowledge about the therapeutic process, for example9

  • Duration of treatment
  • The maximum achievable result
  • The expected onset of the effect
  • Expected adverse events

We also recommend that you offer them the following guidance:1,10

If your patient’s skin peels or becomes dry, they can try

  • Using an oil and fragrance-free hypoallergenic moisturiser
  • Using Duac less often, or stopping for one or two days before starting again

It is important that the prescribed pack size is relative to the extent of acne in order to
optimise application and the treatment response.

DUAC 5% PRESCRIBING INFORMATION

(Please refer to full Summary of Product Characteristics (SPC) before prescribing)
Duac® Once Daily 10mg/g + 50mg/g Gel (clindamycin phosphate 1%, benzoyl peroxide 5%).

Indication: Topical treatment of mild to moderate acne vulgaris, particularly inflammatory lesions. Dosage and administration: Adults and adolescents (aged ≥12 years): Apply thin layer once daily (evening) to affected area after washing. Wash hands after use. Do not exceed more than 12 weeks of continuous use. Contra-indications: Hypersensitivity to clindamycin, lincomycin, benzoyl peroxide or any of the excipients. Precautions: Avoid contact with mouth, eyes, lips, mucous membranes and irritated/broken skin. Caution advised in application to sensitive areas; patients with history of regional enteritis, ulcerative colitis or antibioticassociated colitis; atopic patients in whom further skin drying may occur; and concomitant topical acne therapy. Increased peeling and reddening may occur during the first weeks of treatment. Depending upon the severity, reduce application frequency or temporarily discontinue use. Discontinue if severe local irritancy, prolonged or significant diarrhoea, or abdominal cramps. Sunlamps should not be used and deliberate or prolonged exposure to sun should be avoided or minimised. Sunburn should be resolved before use. Clindamycin resistance: Patients with a recent history of systemic or topical clindamycin or erythromycin use are more likely to have pre-existing anti-microbial resistant Propionibacterium acnes and commensal flora. Cross resistance: May occur with other antibiotics when using antibiotic monotherapy. Avoid contact with hair, fabrics furniture or carpeting due to bleaching. Interactions: Use with caution with concomitant topical antibiotics, medicated or abrasive soaps and cleansers, soaps and cosmetics that have a strong drying effect, and products with high concentrations of alcohol and/or astringents, as a cumulative irritant effect may occur. Caution with concomitant use of neuromuscular blocking agents. Avoid concomitant use with tretinoin, isotretinoin and tazarotene. Do not use in combination with erythromycin-containing products. Concomitant use with topical sulfonamide-containing products may cause skin and facial hair to temporarily change colour (yellow/orange). Fertility, pregnancy and lactation: Safety in human pregnancy and breastfeeding has not been established and should only be prescribed after a careful risk/benefit assessment. Do not apply to breast area if used during lactation. No data on the effect on fertility in humans. Side effects: Erythema, peeling, dryness, pruritus, burning sensation, photosensitivity, headache, application site pain, allergic reactions including anaphylaxis. DERMATOLOGY (EDF) GUIDELINES

High-strength recommendation 		The EDF guidelines for the treatment of acne have given fixed-dose combination BPO/clindamycin and fixed-dose combination BPO/adapalene a high strength recommendation for the treatment of mild to moderate papulopustular acne5,a

OTHER RECOMMENDATIONS:

Medium-strength recommendation 		Azelaic acid; Benzoyl peroxide; Topical retinoidsa; Combination of a systemic antibiotic (doxycycline and lymecyclin, limited to three months) with adapalene, in the case of moderate, more widespread disease; Fixed-dose combination of clindamycin and tretinoin
Low-strength recommendation 		Blue light monotherapy; Fixed-dose combination of erythromycin and tretinoin; Fixed-dose combination of isotretinoin and erythromycin; Oral zinc; combination of a systemic antibiotic with either: azelaic acid; benzoyl peroxide alone or with adapalene in fixed combination (in the case of moderate, more widespread disease)
Negative recommendation 		Topical antibiotics as monotherapy; Artificial UV radiation; Fixed-dose combination of erythromycin and zinc; Systemic therapy with anti-androgens, antibiotics, and/or isotretinoin

Limitations can apply that may necessitate the use of a treatment with a lower strength of recommendation as a first-line therapy (e.g. financial resources/reimbursement limit, legal restrictions, availability, drug licensing).

STUDY DESIGN4 – ZOUBOULIS ET AL. 2009

  • A multicentre, randomised, investigator-blind, parallel-group trial investigating the efficacy, tolerability, and safety of 2 fixed-dose combination gels for the treatment of facial acne: clindamycin 1% + benzoyl peroxide 5% gel (Duac 5%) and adapalene 0.1% + benzoyl peroxide 2.5% gel (Epiduo®)
  • 382 patients entered the study (male and female aged 12-45 years)
    • 190 in the Duac 5% group and 192 in the Epiduo group
    • 337 completed the study
    • The treatment period was 12 weeks and lesion counts, global improvement and time to treatment success were evaluated at baseline and weeks 1,2,4,8 and 12

Primary efficacy endpoint:

  • Percentage change in inflammatory lesion count (papules and pustules, including nasal lesions) from baseline to week 12

Secondary efficacy endpoints:

  • Proportion of participants achieving treatment success☆
  • Time to treatment success from baseline
  • Percentage change in non-inflammatory lesion count from baseline to week 12
  • Percentage change in total lesion count from baseline to week 12
  • Absolute change in lesion count (inflammatory, non-inflammatory and total)
  • Time to 50% reduction in lesion count from baseline (Inflammatory, non-inflammatory and total)

Safety & tolerability endpoints:

  • Local tolerability was assessed using a 5-point scale☆☆
  • Erythema, drying, and peeling were assessed by an investigator
  • Pruritus and burning/stinging were assessed by the participants
  • Adverse events and withdrawals were monitored and classified using the Medical Dictionary for Regulatory Activities (MedDRA) terminology
