IMPLEMENTATION OF THE ASTHMA CONTROL TEST IN A PRIMARY CARE PRACTICE1

OBJECTIVE:

The project aimed to evaluate the implementation of the Asthma Control Test (ACT) in a primary care setting to improve asthma management and outcomes1.

KEY POINTS:

Asthma Control Test (ACT): A brief questionnaire developed in 2004 to assess asthma control and symptoms. It is widely available and takes about one minute to complete1.

Background: Asthma affects339 million people globally, with 50% of adults having uncontrolled asthma1.

Setting: Conducted in a Federally Qualified Health Center in Northern New Jersey. This practice saw a total of 51,237 patients in 2018 over the course of 190,919 visits. In 2018, this practice had 893 patients aged 18 and over with the diagnosis of asthma in 2018 and had 1,397 asthma visits. This equates to 1.56 visits per patient with asthma per year1.

Methodology: A retrospective chart review was performed before and after a brief educational session for medical staff on the use and documentation of the ACT.1

RESULTS:

Educational Impact:

Improved Asthma Management

Implementing the ACT in a primary care setting led to > 70% compliance with the ACT administration and documentation and sustained this compliance for at least 2 years1.

Enhanced Provider Awareness

Medical providers adjusted asthma management plans based on ACT scores of the patients who were identified with uncontrolled asthma, 82.3% had changes made to the asthma treatment plan, which is anticipated to improve their asthma1.

CONCLUSION:

Implementing the ACT in a primary care setting improved asthma control in reducing the healthcare burden and cost and improving the quality of life in patients with asthma1.
